Who Needs to Fill Out DS-11 Form?

You need Form DS-11 if you are applying for a U.S. passport for the first time, you are under 16 years old, your previous passport was issued when you were under 16, your most recent passport was lost or stolen, your most recent passport was issued more than 15 years ago, or your most recent passport was issued more than 5 years ago and you were under 16 when it was issued. If your most recent passport is undamaged, was issued within the last 15 years, and was issued when you were 16 or older, you may be able to renew by mail using Form DS-82 instead.

Penalties for Not Filing DS-11 Form

There are no penalties for submitting Form DS-11, but providing false information on a passport application is a federal crime punishable by up to 10 years in prison and/or a $250,000 fine under 18 U.S.C. 1542.
